How do I make a referral?

 

1. Print the Preliminary Contact sheet and fill in with as much information as possible and submit to one of the above admission clinicians for review.

 

2. If you’re filling out the information for someone else, please ensure you have them sign the release of information, otherwise we will be unable to process any of the information without their consent. The release is a must for everyone including adult parents. It is illegal for Focus to proceed without a release of information signed by the client that gives us permission for their care
